Consent and Acknowledgment

By booking a tattoo appointment at Sickle & Sage Tattoo Collective, you acknowledge and agree to the following terms:

Health Declaration

I confirm that I am 18 years of age or older and in good health. I am not under the influence of alcohol or drugs. I do not have any conditions that may interfere with the tattooing process, including but not limited to hemophilia, diabetes, or skin conditions in the area to be tattooed.

Risks

I understand that tattooing carries risks including, but not limited to, infection, allergic reactions, scarring, and fading. I accept these risks and release Sickle & Sage Tattoo Collective and its artists from liability for any complications that may arise.

Aftercare Responsibility

I understand that proper aftercare is my responsibility. Sickle & Sage Tattoo Collective is not liable for issues arising from improper aftercare. I will follow the aftercare instructions provided by my artist.

Design and Artwork

All custom designs created by our artists are the property of Sickle & Sage Tattoo Collective. Flash designs are one-time use only and will not be tattooed on another client once claimed.

Deposits and Cancellations

Deposits are non-refundable. Appointments require 48 hours notice for rescheduling. Failure to provide adequate notice will result in forfeiture of deposit.
